Thanks for reminding me where the table http://content.onlinejacc.org/cgi/content/full/53/15/e1/TBL3 is. I had seen this before and misplaced it. Nice compendium. Feel free to add link to it from the article. Speaking of ACC/AHA guidelines, did you see:
